This exam is conducted online (computer-based), in which there are total of 200 multiple-choice questions (MCQs). Each question is worth 4 marks if you answer correctly, but if you give a wrong answer, then -1 mark is deducted due to penalty marking, which is called negative marking. Students get 3 hours and 30 minutes to complete this exam.
The syllabus of NEET PG is based on the entire MBBS curriculum, and it covers topics from:
  • Pre-clinical sciences (such as biochemistry, anatomy, and physiology)
  • Para-clinical sciences (such as Pathology, Pharmacology, Microbiology, Forensic Medicine)
  • Clinical sciences (such as Medicine, Surgery, Pediatrics, Obstetrics & Gynecology, ENT, Ophthalmology, etc.)
The exam pattern has changed in recent years. Earlier, many questions were very theoretical, so that they could test your memory and book knowledge. But now, there has been a major change in clinical scenario-based questions. This implies that rather than directly asking for definitions or simple facts, the exam often presents a case study or a Real-Time patient situation. After analyzing the problem and making connections to other subjects, students must find the most accurate solution.
For example, in a single question, students can learn about symptoms (Medicine), diagnostic tests (Pathology/Microbiology), treatment protocols (Pharmacology), and even surgical options (Surgery). Students will not only understand the information but also learn how to apply it, just like in real doctor practice.
If students want to crack NEET PG, then they need to know some specific learning.
They must focus on:
  • Conceptual clarity of all subjects
  • Understanding of clinical cases
  • An interdisciplinary approach, when expertise from many fields is combined

Mock Test Strategy 2026

You should first start by focusing on preparing the mock tests subject-wise. This will strengthen your roots. Start with your strong subjects initially and then slowly move towards difficult subjects.
During this phase, your main goal is understanding all the questions and patterns, not just answering as many questions as you can.
You need to take one mock test every three days, and spend an equal amount of time reviewing every answer carefully. Understand all the questions completely, and solve and understand the rating of every question that you have.
After that, make a detailed analysis sheet so that you can track your performance. Create sections for each topic, question types, and difficulty levels. This will help you to recognize patterns of mistakes and find your improvement areas.
Also, this helps ensure that your basics are rock solid, which will make later phases much easier.
Once your foundation is strong, and you understand all the subjects clearly. After that, you will take full-length mock tests, which are exactly like the NEET PG exam format. This will further improve your fundamentals. In this phase, you will especially learn clinical correlation to understand how to handle real-life medical situations.

As you improve, increase your mock test frequency to every alternate day, and also keep track of your performance analysis sheet and manage it better. In this phase, you need to keep applying your knowledge rather than just memorization, think in your mind about real-life situations, what problems can arise, and how you will solve them. You should be connecting concepts from different subjects to solve questions, which is crucial for scoring well.

You will be able to achieve success in this phase only through your consistency. The more you practice, the more you will chase perfection. During this, you will have to do daily full-length mock tests from a variety of sources to cover as many question patterns as possible. You will have to remove all small and big distractions, and along with this, you will also have to balance your stress; otherwise, you may get confused due to stress in so many things.

At this stage, it is most important that you understand the improvement and where you need to improve now, because at this stage, you do not see big things to improve, so you need to understand every small issue and improve it. You will have to understand your every weakness and strategy for each subject and question type.
If you manage your time, accuracy, remove all distractions, and keep preparing consistently, then you will easily be ready for the exam.
